كشفت دراسات طبية حديثة عن نتائج لافتة في ثلاثة مجالات مختلفة، إذ ارتبط استئصال الزائدة الدودية بانخفاض خطر الإصابة بسرطان القولون والمستقيم، بينما رصد باحثون أعراضًا غير شائعة لدى بعض مستخدمي أدوية إنقاص الوزن والسكري من نوع GLP-1، في وقت أظهر علاج تجريبي بالخلايا نتائج واعدة لمرضى يعانون من متلازمة الشخص المتصلب، المرض الذي أعلنت المغنية سيلين ديون إصابتها به.
استئصال الزائدة وسرطان القولون
وأظهرت دراسة شملت أكثر من 30 ألف حالة أن المرضى الذين خضعوا لاستئصال الزائدة الدودية الملتهبة كانوا أقل عرضة لتشخيص سرطان القولون والمستقيم مقارنة بمن عولجوا بالمضادات الحيوية فقط.
وراجع الباحثون بيانات 15,774 مريضًا بالتهاب الزائدة الدودية الحاد خضعوا للجراحة، مقابل 15,295 مريضًا تلقوا المضادات الحيوية دون استئصال الزائدة.
وخلال المتابعة، شُخص سرطان القولون والمستقيم لدى 0.7% من المرضى الذين خضعوا للجراحة، مقارنة بـ 1.7% ممن عولجوا بالمضادات الحيوية.
وبعد احتساب عوامل الخطورة الفردية، ارتبط استئصال الزائدة بانخفاض نسبي قدره 58% في خطر الإصابة اللاحقة بسرطان القولون والمستقيم.
وعُرضت النتائج خلال المؤتمر السريري للكلية الأمريكية للجراحين في واشنطن.
وقال الباحث الرئيسي الدكتور فالنتين نفونسام، من كلية مورهاوس للطب في أتلانتا، إن علاج التهاب الزائدة شهد تغيرًا كبيرًا خلال العقد الماضي، بعدما أصبحت المضادات الحيوية خيارًا مقبولًا إلى جانب الجراحة.
وتشير أبحاث حديثة إلى أن الزائدة الدودية قد يكون لها دور في جهاز المناعة المعوي وميكروبيوم الأمعاء، بعدما كان يُنظر إليها طويلًا باعتبارها عضوًا بلا وظيفة مهمة، وقد يكون الالتهاب المستمر أو التغيرات في ميكروبيوم الأمعاء مرتبطًا بتطور السرطان، لكن الدراسة الحالية لم تثبت هذا التفسير أو علاقة سببية.
وأكد نفونسام أن النتائج لا تعني أن استئصال الزائدة يجب أن يُجرى بهدف الوقاية من السرطان، مشددًا على أهمية العلاج بالمضادات الحيوية والمتابعة المناسبة عند اختيار العلاج غير الجراحي.
أعراض جديدة مع أدوية التخسيس
وفي دراسة أخرى، رصد باحثون زيادة في حالات انفصال الأظافر لدى مستخدمي أدوية GLP-1 المستخدمة لإنقاص الوزن وعلاج السكري.
وشملت الدراسة 575 شخصًا بدأوا استخدام هذه الأدوية للمرة الأولى، و1,329 شخصًا لم يستخدموها من قبل، وكان المشاركون يعانون من السكري أو السمنة أو كليهما.
وأفاد 39% من مستخدمي GLP-1 بانفصال الأظافر، مقابل 9% فقط من غير المستخدمين، واستمر ارتفاع الخطر بغض النظر عن مقدار الوزن الذي فقده المشاركون.
وفي دراسة منفصلة على المجموعة نفسها، أبلغ 26% من مستخدمي الأدوية عن تساقط الشعر، مقابل 12% من غير المستخدمين، بينما أشار 42% إلى انخفاض حجم الشعر أو كثافته، مقارنة بـ 19% في المجموعة الأخرى.
كما عانى معظم المصابين بتساقط الشعر من أعراض في فروة الرأس، بينها الحكة والاحمرار والتقشر.
وقال طبيب الأمراض الجلدية الفرنسي برونو هاليو إن هذه النتائج قد تشير إلى أن التغيرات في فروة الرأس تتجاوز التساقط التقليدي المرتبط بفقدان الوزن السريع، وربما تتضمن عنصرًا التهابيًا قابلًا للعلاج.
ومع ذلك، لا تثبت الدراستان أن أدوية GLP-1 تسببت مباشرة في تغيرات الأظافر أو الشعر، وإنما تشير النتائج إلى احتمال وجود عوامل أخرى إلى جانب فقدان الوزن.
علاج تجريبي لمتلازمة الشخص المتصلب
وفي تطور آخر، أظهر العلاج التجريبي mivocabtagene autoleucel (miv-cel)، نتائج واعدة لدى مرضى يعانون من اضطرابات مناعية ذاتية شديدة.
وأظهرت تجربة متقدمة تحسنًا ملحوظًا في القدرة على المشي ومؤشرات أخرى لدى مرضى متلازمة الشخص المتصلب، مع استمرار الفوائد بعد عام من العلاج.
وتسبب المتلازمة تيبسًا عضليًا وزيادة الحساسية للأصوات واللمس والمؤثرات العاطفية، ما قد يؤدي إلى تشنجات عضلية، وكانت سيلين ديون أعلنت إصابتها بالمرض عام 2022.
ويُعد miv-cel نوعًا من العلاج بالخلايا التائية المعدلة CAR T، إذ تُسحب خلايا T من المريض وتُعدل وتُكثّر في المختبر لاستهداف المرض، ثم تُعاد إلى الجسم. وكانت أول علاجات CAR T المعتمدة مخصصة لسرطانات الدم.
وفي تجربة منفصلة متوسطة المرحلة، أدى العلاج أيضًا إلى تحسن الأعراض والقدرة على أداء الأنشطة اليومية لدى مرضى الوهن العضلي الوبيل المعمم، الذي يسبب ضعفًا متقلبًا في العضلات الهيكلية.
وتجري حاليًا تجربة متقدمة للعلاج في مرضى الوهن العضلي، ومن المتوقع اكتمال تسجيل المشاركين بحلول منتصف 2027.
Recent medical studies have revealed striking results in three different areas. The removal of the appendix has been linked to a decreased risk of colon and rectal cancer, while researchers have observed unusual symptoms in some users of GLP-1 weight loss and diabetes medications. At the same time, an experimental cell therapy has shown promising results for patients suffering from stiff person syndrome, the condition that singer Celine Dion announced she has.
Appendectomy and Colon Cancer
A study involving more than 30,000 cases showed that patients who underwent appendectomy for an inflamed appendix were less likely to be diagnosed with colon and rectal cancer compared to those who were treated with antibiotics alone.
The researchers reviewed data from 15,774 patients with acute appendicitis who underwent surgery, compared to 15,295 patients who received antibiotics without appendectomy.
During the follow-up, colon and rectal cancer was diagnosed in 0.7% of the patients who underwent surgery, compared to 1.7% of those treated with antibiotics.
After accounting for individual risk factors, appendectomy was associated with a relative reduction of 58% in the subsequent risk of developing colon and rectal cancer.
The results were presented at the clinical conference of the American College of Surgeons in Washington.
The lead researcher, Dr. Valentin Nfonsam from Morehouse School of Medicine in Atlanta, stated that the treatment of appendicitis has undergone significant changes over the past decade, with antibiotics becoming an acceptable option alongside surgery.
Recent research suggests that the appendix may play a role in the gut immune system and the gut microbiome, after being long viewed as an organ without an important function. Chronic inflammation or changes in the gut microbiome may be linked to cancer development, but the current study did not prove this interpretation or a causal relationship.
Nfonsam emphasized that the results do not mean that appendectomy should be performed for cancer prevention, stressing the importance of antibiotic treatment and appropriate follow-up when choosing non-surgical treatment.
New Symptoms with Weight Loss Medications
In another study, researchers observed an increase in cases of nail separation among users of GLP-1 medications used for weight loss and diabetes treatment.
The study included 575 individuals who started using these medications for the first time and 1,329 individuals who had never used them before, with participants suffering from diabetes, obesity, or both.
39% of GLP-1 users reported nail separation, compared to only 9% of non-users, and the increased risk persisted regardless of the amount of weight lost by the participants.
In a separate study of the same group, 26% of medication users reported hair loss, compared to 12% of non-users, while 42% noted a decrease in hair volume or density, compared to 19% in the other group.
Most of those experiencing hair loss reported symptoms on the scalp, including itching, redness, and flaking.
French dermatologist Bruno Halio stated that these results may indicate that changes in the scalp go beyond the traditional hair loss associated with rapid weight loss and may involve an inflammatory component that is treatable.
However, the two studies do not prove that GLP-1 medications directly caused changes in nails or hair, but the results suggest the possibility of other factors alongside weight loss.
Experimental Treatment for Stiff Person Syndrome
In another development, the experimental treatment mivocabtagene autoleucel (miv-cel) has shown promising results in patients with severe autoimmune disorders.
A late-stage trial demonstrated significant improvement in walking ability and other indicators in patients with stiff person syndrome, with benefits continuing after one year of treatment.
The syndrome causes muscle stiffness and increased sensitivity to sounds, touch, and emotional stimuli, which can lead to muscle spasms. Celine Dion announced her diagnosis of the condition in 2022.
Miv-cel is a type of CAR T cell therapy, where T cells are extracted from the patient, modified, and multiplied in the lab to target the disease, then returned to the body. The first approved CAR T therapies were designed for blood cancers.
In a separate mid-stage trial, the treatment also led to improvements in symptoms and the ability to perform daily activities in patients with generalized myasthenia gravis, which causes fluctuating weakness in skeletal muscles.
An advanced trial of the treatment is currently underway in myasthenia gravis patients, with participant enrollment expected to be completed by mid-2027.
