A deficiency in vitamin B12 is considered one of the nutritional issues that may go unnoticed for a long time, as its symptoms develop gradually in some individuals, and many of them may resemble symptoms of other health conditions.
Vitamin B12 plays essential roles in the formation of red blood cells and maintaining the health of the nervous system. Therefore, its deficiency, if left untreated, can lead to various physical and neurological symptoms.
The British National Health Service (NHS) indicates that some neurological problems resulting from vitamin deficiency may become permanent if their discovery and treatment are delayed.
According to Hindustan Times, anesthesiologist and interventional pain specialist Dr. Kunal Sood explains the various symptoms that may indicate a vitamin B12 deficiency.
He clarified that these symptoms do not necessarily mean a deficiency in vitamin B12, but feelings of tingling, changes in the tongue, fatigue, cognitive disturbances, or walking difficulties that occur together warrant evaluation. He pointed out that a blood test can confirm the deficiency, and early treatment is important because long-term nerve damage may not completely resolve.
He noted five subtle signs that may reveal a vitamin B12 deficiency, which are:
1 - Smooth and painful tongue
Changes in the shape or sensation of the tongue may be signs worth noting; the tongue may become red, painful, or smoother than usual, which are symptoms that can be associated with B12 deficiency.
This condition is known as atrophic glossitis, and some affected individuals may also develop mouth ulcers.
Medical guidelines indicate that glossitis can occur with B12 deficiency even in the absence of anemia.
2 - Numbness or tingling sensation
Persistent feelings of numbness or tingling, especially in the hands and feet, are among the most prominent neurological symptoms associated with vitamin B12 deficiency.
The vitamin plays a role in maintaining the integrity of the nervous system, and its deficiency may lead to peripheral neuropathy, causing sensations of tingling, loss of sensation, or muscle weakness.
These symptoms may appear even when there is no obvious anemia, but numbness alone does not necessarily indicate a B12 deficiency, as it can have many causes; therefore, it should be evaluated in its medical context and not assumed to be directly due to vitamin deficiency.
3 - Unexplained fatigue and general weakness
B12 deficiency can manifest as persistent fatigue or a noticeable decrease in energy, especially when the deficiency leads to anemia.
The body needs B12 to help produce red blood cells, and its deficiency can lead to megaloblastic anemia, which can cause fatigue, weakness, dizziness, shortness of breath, and decreased ability to engage in physical activity.
Since fatigue is a common symptom with many causes, it is not sufficient alone to diagnose vitamin deficiency.
4 - "Brain fog" and memory changes
B12 deficiency may also affect the nervous system and cognitive functions, which may present as difficulty concentrating, slow thinking, forgetfulness, or mood changes, with severe cases potentially leading to confusion.
The NHS indicates that memory, comprehension, and judgment issues can appear in individuals with B12 deficiency, whether or not accompanied by anemia.
5 - Balance issues and difficulty walking
One of the less expected signs of B12 deficiency is problems with balance and motor coordination.
Continuous deficiency may affect the neural pathways responsible for sensing limb position, which can cause instability while walking, stumbling and falling, difficulty walking in the dark, or changes in gait.
Medical guidelines list balance problems, coordination issues, and gait disturbances among the potential neurological symptoms of B12 deficiency.
When should a B12 test be conducted?
The presence of one of these symptoms does not automatically mean that a person has a vitamin B12 deficiency, as the same symptoms can arise from different causes. However, the presence of multiple symptoms, especially numbness, memory changes, or balance issues along with fatigue or tongue changes, warrants discussion with a doctor.
Diagnosis of B12 deficiency typically relies on symptoms and medical history, along with blood tests that measure the vitamin level, with other tests that may help determine the cause if needed.
Why should neurological symptoms not be ignored?
The importance of early detection of B12 deficiency lies in the fact that treatment can significantly improve many symptoms, but some damage to the nervous system may become irreversible if the deficiency remains untreated for a long time.
Additionally, the causes of B12 deficiency are varied and are not limited to diet; they may be related to absorption issues, certain gastrointestinal diseases, previous surgeries on the stomach or intestines, or some medications such as metformin and certain proton pump inhibitors.
