يتبادر إلى أذهان البعض، أن «طبيب التخدير» هو ذلك الذي يعطي المريض دواء لينام قبل العملية ثم يوقظه بعد انتهائها، صورة تختصر تخصصاً كاملاً في لحظتين، بينما ما يحدث بينهما يحمل مسؤولية أكبر بكثير، فطبيب التخدير لا يتعامل مع «التخدير» وحده، بل يتولى رعاية المريض في واحدة من أكثر مراحل علاجه حساسية.
تبدأ مهمته قبل الجراحة بتقييم الحالة الصحية للمريض، ومراجعة أمراضه وأدويته، وتقدير عوامل الخطورة، واختيار خطة التخدير المناسبة والاستعداد لما قد يطرأ، فليست كل عملية متشابهة، ولا كل استجابة للأدوية متوقعة، وما يناسب مريضاً قد لا يناسب آخر.
وأثناء الجراحة، تبدأ مرحلة لا يراها المريض غالباً، ففي الوقت الذي ينشغل فيه الجرَّاح بإجراء العملية يتابع طبيب التخدير التنفس ومجرى الهواء والدورة الدموية والسوائل والأدوية والسيطرة على الألم، ويتعامل مع تغيرات قد تحدث خلال دقائق أو حتى ثوان.
وهنا يظهر جانب آخر لهذا التخصص، فهو لا يعتمد على المعرفة الطبية والمهارات الإجرائية فحسب، بل يتطلب التخطيط وترتيب الأولويات وسرعة الاستجابة للمتغيرات والتواصل المستمر مع الفريق الجراحي.
ولا تنتهي مسؤولية طبيب التخدير بانتهاء العملية، بل تمتد إلى مرحلة الإفاقة والسيطرة على الألم والتأكد من استقرار المريض، كما يمتد حضور هذا التخصص إلى مجالات أخرى، تشمل الإنعاش والرعاية الحرجة وعلاج الألم.
ربما لا يتذكر المريض طبيب التخدير بعد أن يستيقظ، لكنه كان حاضراً في كل لحظة لم يكن المريض قادراً فيها على مراقبة نفسه، فالتخدير علم الحفاظ على سلامة المريض حين يضع حياته مؤقتاً بين أيدي فريق لا يراه.
Some may think that the "anesthesiologist" is simply the one who gives the patient medication to sleep before the surgery and then wakes them up afterward, a portrayal that condenses an entire specialty into two moments. However, what happens between these moments carries a much greater responsibility, as the anesthesiologist does not deal with "anesthesia" alone, but rather takes care of the patient during one of the most sensitive stages of their treatment.
Their task begins before the surgery with an assessment of the patient's health, reviewing their medical history and medications, estimating risk factors, selecting the appropriate anesthesia plan, and preparing for any unforeseen events. Not every surgery is the same, nor is every response to medications predictable; what works for one patient may not work for another.
During the surgery, a phase often unseen by the patient begins. While the surgeon is busy performing the operation, the anesthesiologist monitors breathing, airway, circulation, fluids, medications, and pain control, addressing changes that may occur within minutes or even seconds.
Here, another aspect of this specialty emerges; it relies not only on medical knowledge and procedural skills but also requires planning, prioritization, quick response to changes, and continuous communication with the surgical team.
The anesthesiologist's responsibility does not end with the completion of the surgery; it extends to the recovery phase, pain management, and ensuring the patient's stability. The presence of this specialty also extends to other areas, including resuscitation, critical care, and pain treatment.
The patient may not remember the anesthesiologist after waking up, but they were present in every moment when the patient was unable to monitor themselves. Anesthesia is the science of keeping the patient safe when they temporarily place their life in the hands of a team they cannot see.
