Two large international studies have revealed that a range of blood tests and non-invasive imaging techniques can detect advanced fibrosis and cirrhosis of the liver with a high degree of accuracy in patients with metabolic-associated fatty liver disease, potentially reducing the need for liver biopsy in several cases.
More than 1500 patients
The studies were part of the international "LITMUS" program, which is one of the largest research efforts to evaluate diagnostic methods for metabolic-associated fatty liver disease (MASLD) and its related condition, metabolic-associated steatotic hepatitis (MASH).
The main study, published in the journal "The Lancet," included over 1500 patients and compared the performance of a wide range of blood tests and ultrasound-based liver stiffness measurements with liver biopsy results examined by pathologists.
High accuracy in MRI imaging
The second study, published in the journal "Nature Medicine," investigated advanced imaging techniques, including magnetic resonance elastography (MRE) and stiffness measurement techniques using "FibroScan," in more than 350 patients.
The results showed that magnetic resonance elastography, along with several "FibroScan" tests and protein analyses, achieved high levels of accuracy in detecting cases of advanced fibrosis and cirrhosis.
A simple blood test outperforms a common test
One of the key findings was that the simple and low-cost enzyme "Gamma-GT" test outperformed the widely used "FIB-4" index as a primary screening test for assessing the risk of liver fibrosis.
The researchers noted the potential to use this analysis as a first step, followed by more specialized tests such as "FibroScan" or the "ELF" test when a more precise assessment of disease severity is needed.
Tests excel in detecting cirrhosis
The protein techniques "SOMAScan," magnetic resonance elastography, and "FibroScan" tools, including the "Agile 3+" index, recorded the best performance levels in detecting advanced fibrosis and cirrhosis of the liver.
Additionally, the blood tests "ELF" and "ADAPT" exceeded the previously defined thresholds for the accuracy required in detecting liver cirrhosis.
Biopsy will not disappear completely
Despite the promising results, the researchers emphasized that non-invasive tests are not yet capable of replacing biopsy in all cases, as diagnosing active inflammation associated with "MASH" remains more challenging.
The researchers stated that the results provide a foundation for developing a diagnostic pathway that starts with simple and low-cost tests, then progresses to imaging techniques or more advanced examinations when necessary, potentially reducing the number of patients who require liver biopsy.
Metabolic-associated fatty liver disease affects up to one in three adults globally, with incidence rates significantly higher among those with obesity or type 2 diabetes.
