يُعد تناول فيتامين D بالتزامن مع أدوية ضغط الدم آمناً لدى معظم الأشخاص، إلا أن نوع دواء الضغط وجرعة الفيتامين، إلى جانب استخدام مكملات الكالسيوم، قد تؤثر في احتمالات ظهور بعض الآثار الجانبية.
ولا يُعرف عن فيتامين D وجود تفاعلات مؤثرة مع معظم الأدوية الشائعة المستخدمة لعلاج ارتفاع ضغط الدم، لذلك يمكن لمن أوصى الطبيب لهم بتناول مكملاته بسبب انخفاض مستوياته الاستمرار عادة في استخدام أدوية الضغط وفق الجرعات الموصوفة.
لكن الاستثناء الأبرز يتعلق بمدرات البول من فئة «الثيازيد»، التي تشمل أدوية مثل هيدروكلوروثيازيد وكلورثاليدون وإندباميد، إذ تعمل هذه الأدوية على خفض ضغط الدم من خلال زيادة طرح الصوديوم والماء من الجسم، لكنها في الوقت نفسه تقلل فقدان الكالسيوم.
وبما أن فيتامين D يعزز امتصاص الكالسيوم من الجهاز الهضمي، فإن تناول جرعات مرتفعة منه بالتزامن مع مدرات البول الثيازيدية قد يؤدي في بعض الحالات إلى ارتفاع مستوى الكالسيوم في الدم، خصوصاً لدى من يستخدمون مكملات الكالسيوم أيضاً، أو المصابين بأمراض الكلى أو فرط نشاط الغدة الجار درقية، إضافة إلى كبار السن.
وقد يلجأ الأطباء إلى متابعة مستويات الكالسيوم بشكل دوري لدى من يجمعون بين هذه الأدوية ومكملات فيتامين D، ولا سيما عند استخدام جرعات مرتفعة.
وتتطلب المكملات المركبة التي تحتوي على فيتامين D والكالسيوم معاً مزيداً من الانتباه، إذ قد تكون مفيدة لبعض الأشخاص المعرضين لهشاشة العظام، لكنها قد تزيد احتمال ارتفاع الكالسيوم عند استخدامها مع مدرات البول الثيازيدية.
ولهذا يُنصح بعدم تناول فيتامين D والكالسيوم معاً بصورة تلقائية لمن يستخدم هذه الأدوية، وضرورة الرجوع إلى الطبيب لتحديد الحاجة الفعلية لكل مكمل وفق النظام الغذائي والحالة الصحية ونتائج الفحوص.
ومن أبرز الأعراض التي قد ترافق ارتفاع الكالسيوم في الدم الغثيان أو التقيؤ، والإمساك، وزيادة العطش، وكثرة التبول، وضعف العضلات، والإرهاق، إضافة إلى التشوش الذهني، وهي أعراض ترتبط غالباً بالإفراط في تناول فيتامين D أكثر من ارتباطها بالجرعات التعويضية المعتادة.
وفي ما يتعلق بدور فيتامين D في خفض ضغط الدم، تشير بعض الدراسات إلى وجود ارتباط بين نقص الفيتامين وارتفاع الضغط، مع اعتقاد الباحثين بأن تصحيح النقص قد يسهم بصورة غير مباشرة في تنظيم بعض الآليات المرتبطة بالضغط.
إلا أن نتائج التجارب السريرية على مكملات فيتامين D جاءت متباينة، إذ لم تُظهر مراجعات واسعة تأثيراً واضحاً في خفض ضغط الدم لدى عموم الأشخاص، خصوصاً ممن لديهم مستويات طبيعية من الفيتامين.
وعليه، لا يُعد فيتامين D علاجاً لارتفاع ضغط الدم، ولا ينبغي إيقاف الأدوية الموصوفة أو تعديل جرعاتها عند بدء تناول المكمل إلا بناءً على توجيه الطبيب.
وعادة ما تتراوح الجرعات اليومية المستخدمة لتعويض النقص بين 800 و2000 وحدة دولية، وقد تُستخدم جرعات أعلى بصورة مؤقتة في حالات النقص الحاد، إلا أن تناول جرعات مرتفعة لفترات طويلة قد يؤدي إلى زيادة الكالسيوم في الدم ورفع خطر الإصابة بحصى الكلى ومضاعفات أخرى.
وينصح المختصون بإجراء فحص لمستوى فيتامين D، ومناقشة الجرعة المناسبة مع الطبيب، خصوصاً قبل تناول جرعات مرتفعة أو عند استخدام أدوية ضغط الدم ومدرات البول.

Doctors may monitor calcium levels periodically in those combining these medications with vitamin D supplements, particularly when using high doses.
Combined supplements containing both vitamin D and calcium require more attention, as they may be beneficial for some individuals at risk of osteoporosis, but they may increase the likelihood of elevated calcium levels when used with thiazide diuretics.
Therefore, it is advised not to take vitamin D and calcium together automatically for those using these medications, and it is essential to consult a doctor to determine the actual need for each supplement based on dietary intake, health status, and test results.
Some of the most prominent symptoms that may accompany elevated calcium levels in the blood include nausea or vomiting, constipation, increased thirst, frequent urination, muscle weakness, fatigue, and mental confusion, which are symptoms often associated with excessive intake of vitamin D rather than with the usual compensatory doses.
Regarding the role of vitamin D in lowering blood pressure, some studies suggest a link between vitamin deficiency and high blood pressure, with researchers believing that correcting the deficiency may indirectly contribute to regulating some mechanisms associated with blood pressure.
However, the results of clinical trials on vitamin D supplements have been varied, as extensive reviews have not shown a clear effect in lowering blood pressure in the general population, especially among those with normal vitamin levels.
Thus, vitamin D is not considered a treatment for high blood pressure, and prescribed medications should not be stopped or their doses adjusted upon starting the supplement except under a doctor's guidance.
