Obesity is no longer just an issue related to appearance or lifestyle habits; it has become one of the most significant public health challenges in Saudi Arabia due to its direct link to rising rates of chronic diseases.
According to the latest data from the General Authority for Statistics, 23.1% of Saudis aged 15 and older suffer from obesity, while 45.1% are overweight.
Health authorities confirm that obesity increases the risk of developing type 2 diabetes, high blood pressure, and cardiovascular diseases, and is associated with a higher likelihood of several types of cancer. Its effects are not limited to these conditions; they extend to affect women's reproductive health and pregnancy complications, as well as negatively impacting quality of life and mental health.
Amid these alarming health indicators and the daily reality faced by thousands of women, the investigation by "Okaz" aims to explore the main factors contributing to the spread of obesity among women in Saudi Arabia, examine its health and social repercussions, and highlight the most important solutions and strategies that can help reduce its prevalence and future effects.
The Impact on Mental Health
Family medicine consultant Dr. Badr Madani believes that obesity is a chronic disease in itself, not merely a risk factor, explaining that it is associated with hormonal, metabolic, and genetic disorders, alongside the influence of environmental and behavioral factors.
He added that the most common diseases among those suffering from obesity include type 2 diabetes, high blood pressure, lipid and cholesterol disorders, arthritis, cardiovascular diseases, sleep apnea, and non-alcoholic fatty liver disease. It also increases the likelihood of developing certain types of cancer, such as breast, colon, and endometrial cancer, in addition to its impact on mental health.
Dr. Madani noted a decrease in the average age of those suffering from chronic diseases, indicating that type 2 diabetes and high blood pressure are increasingly prevalent among young people, even teenagers, due to the rise in obesity and lack of physical activity, which means exposure to health complications at an early age.
He emphasized the importance of early detection and the necessity of regular check-ups for individuals suffering from obesity or having risk factors, including measuring blood pressure, fasting and HbA1c glucose tests, cholesterol levels, and liver and kidney functions, in addition to assessing fatty liver disease and sleep apnea, and conducting hormonal tests for women experiencing menstrual irregularities.
The Family Doctor as the First Line of Defense
Dr. Madani explains that the family doctor represents the first line of defense in preventing obesity and its complications by regularly monitoring weight, providing nutritional and behavioral guidance, early detection of associated diseases, and referring the patient to the appropriate specialist when necessary, stressing that treating obesity requires continuous follow-up and a comprehensive approach, not just relying on diet alone.
He pointed out that losing between 5% and 10% of body weight achieves significant health benefits, including improving insulin sensitivity, lowering blood pressure and fats, improving fatty liver and joint pain, and reducing the risk of diabetes, even without reaching the ideal weight.
Dr. Madani concluded his remarks by emphasizing that one of the most common mistakes patients make is following extreme diets, relying on exercise without modifying their diet, neglecting sleep and stress management, and using medications or supplements without medical supervision, asserting that success in treating obesity begins with making balanced and sustainable lifestyle changes.
It’s Not Just About Food Quantity
In the opinion of clinical nutrition consultant Dr. Shawq Al-Ashmali, obesity among women continues to pose a health challenge despite increasing awareness and expanding prevention and treatment programs. Women are among the most affected groups, especially after pregnancy and childbirth, as well as with aging and menopause.
She explained that obesity results from a combination of multiple factors, including lack of physical activity, reliance on high-calorie meals, sleep disturbances, psychological stress, repeated pregnancies, certain health issues, and medications, emphasizing that the problem is not linked to willpower as much as it is to biological, behavioral, and environmental factors.
She added that weight gain is not only related to the quantity of food but also to the quality of food and lifestyle, noting that sugary drinks and snacks can add high calories without feeling full, while physical activity, especially resistance training, helps maintain muscle mass and improve health even with slow weight loss.
Threatening Fertility and Increasing Pregnancy Risks
Dr. Saeed Bardwan, an assistant consultant in obstetrics and gynecology at King Faisal Specialist Hospital, confirmed that obesity directly affects women's reproductive health, leading to hormonal imbalance due to increased insulin resistance and elevated estrogen levels, which impacts ovulation and menstrual regularity, increasing the likelihood of delayed conception, miscarriage, and complications during pregnancy and childbirth.
He explained that there is a close relationship between obesity and polycystic ovary syndrome, as weight gain exacerbates ovulatory disorders, elevated male hormone levels, and irregular menstrual cycles, noting that losing between 5% and 10% of body weight may improve menstrual regularity and increase ovulation chances.
He pointed out that obesity is one of the most modifiable factors affecting fertility, as it reduces the chances of natural conception and is associated with lower success rates in assisted reproductive treatments and higher miscarriage rates, asserting that improving weight before starting fertility treatments contributes to increasing success chances.
He added that obesity during pregnancy increases the risk of gestational diabetes, high blood pressure, preeclampsia, blood clots, cesarean delivery, as well as the likelihood of miscarriage, preterm birth, and delivering babies with high birth weights, in addition to increasing the risk of certain congenital anomalies.
Address It Before Pregnancy
Dr. Bardwan pointed out that obesity is one of the most significant risk factors for endometrial cancer and increases the risk of breast cancer after menopause due to elevated estrogen levels produced by fat tissues, explaining that the risk of endometrial cancer significantly increases with a higher body mass index.
He concluded his remarks by emphasizing that losing weight before planning for pregnancy is one of the most important preventive steps, as it improves fertility, reduces complications during pregnancy and childbirth, and enhances the health of both mother and fetus, stressing that the goal is to achieve sustainable health improvement through a balanced lifestyle, not to pursue rapid weight loss.
Avoiding Carbs... Is It Beneficial?
Dr. Shawq Al-Ashmali noted that one of the most common misconceptions is the belief that one must completely avoid carbohydrates, or that extreme diets yield better results, or that there are products that burn fat, emphasizing that losing 5% to 10% of body weight is sufficient to achieve a noticeable improvement in blood pressure, glucose and fat levels, and quality of life.
She confirmed that obesity increases the risk of type 2 diabetes, high blood pressure, heart diseases, and fatty liver, and is scientifically linked to a higher likelihood of several cancers, most notably endometrial cancer, breast cancer after menopause, and colon cancer.
Dr. Al-Ashmali concluded by emphasizing that prevention starts with following a balanced lifestyle, which includes healthy eating, regular physical activity, sufficient sleep, reducing sedentary behavior, and early medical follow-up when needed, stressing that obesity is a chronic disease that requires a sustainable treatment plan, not quick fixes or temporary diets.
